The Civitan-Sparks Clinics, located in the Community Health Services building on 20th St. S., were established as the Sparks Center for Developmental and Learning Disorders. Civitan-Sparks Clinics provide many essential clinical and community services for individuals and families. The clinics provide evaluation and intervention services for individuals with a broad range of physical, developmental, learning and/or behavioral challenges, in a family-centered and culturally-sensitive setting.
An interdisciplinary team made up of many kinds of professionals work closely to meet the needs of individuals and their families. Customized team evaluations address the areas of concern and provide recommendations for therapies, treatment, and activities that will help each individual achieve their full potential and live a meaningful life.
The clinics are part of the Civitan international Research Center whose mission is to provide education research and services on developmental disabilities.

The intake form is also available on this website for you to complete and mail to us at your convenience. Telephone assistance is available if needed to complete the form. This information is used to schedule customized appointments for the appropriate clinics and specialists.
In addition to doctor and other professional referrals individual and family referrals are welcome.
Individual and/or group health insurance and Medicaid are accepted.
