Addiction Recovery Assessment
If you or someone you love is concerned about a potential substance use disorder, the UAB Medicine Addiction Recovery Program can help. The self-assessment below allows potential patients to review common signs and symptoms of addiction. Self-assessment
- Do you feel that you have a problem with alcohol or drugs?
- Are you ever preoccupied with thoughts of drinking or using?

- If you drink or use, can you stop after one glass or one hit?

- Do you rely on drinking or using as a coping mechanism?
Sexual addiction, sexual anorexia, love addiction, and relationship addiction are complex issues that require a thorough analysis to make a proper diagnosis. These issues often impact a person’s recovery from substance use disorder and may require separate treatment.

Sex Addiction Screening
- Have you experienced difficulty resisting impulses to engage in sexual behaviors?
- Have you tried to stop, control, or reduce these behaviors?
- Have you thought of killing yourself because of your sexual behaviors?
- Have you experienced legal consequences due to your sexual behaviors?
- Do you spend large amounts of time trying to get sex or recover from being sexual?
- Do you ever feel anxious or irritable if you are unable to engage in sexual behaviors?
- Do you worry that others will find out about your sexual activities?
- Do you often find yourself preoccupied with sexual thoughts?
- Do you feel that your sexual behavior is not normal?
- Are you experiencing family problems as a result of your behaviors?
Evaluations by the UAB Medicine Addiction Recovery Program are available to patients applying to the professionals program for addiction treatment. The assessment will help potential patients, people who refer patients to our program, and organizations make clinically sound decisions about whether addiction treatment is needed and what level of care the individual may require.
We also offer these evaluations to those who may have needs related to custody, employment, or college education. The evaluation has been designed to explore particular issues identified by the concerned parties and provide the patient and referring parties with comprehensive conclusions and recommended courses of action. Following the assessment, diagnoses and recommendations are presented and provided to the referral source in a detailed report.
Our evaluation team for addiction treatment includes:
- Attending Physician
- Counselor
- Addiction Medicine Specialists
- Registered nurse
Each evaluation may include:
- Comprehensive history & physical examination
- Psychiatric evaluation
- Biopsychosocial evaluation
- Lab work and drug testing
- Comprehensive psychological testing
- Cognitive testing
- Collateral Contacts
- Group processing
Evaluation Process
- When appropriate, basic information about the evaluation is given to the individual, and basic information is gathered from the individual and the referral source.
- The individual arrives, initial paperwork is completed, and lab work is done.
- Each step in the evaluation process described above is completed within an individual meeting. These take place over a period of 3-5 days.
- Collateral information is gathered.
- Individual assessments are completed, and the principal members of the team meet to discuss the findings.
- The evaluation team makes its diagnoses and recommendations.
- A written and signed document describing the team’s initial diagnoses and recommendations is sent to the referral source and verbally communicated to the individual during the exit interview.
- Within 2-3 weeks, a full report is sent to the referral source and made available to the individual upon request. The report contains all of the individual assessments, the collateral information, a summary of findings, and final diagnostic impressions and recommendations.
